Our Fee Structure: A Flexible, Person-Centered Approach

- Level of assessment required (how in-depth, cognitive, home visits vs remote, etc.)
- Frequency of ongoing coordination (e.g. number of check-ins, provider communications)
- Specialized services needed (memory care, home therapies, palliative care, specialized equipment, etc.)
- Travel or transportation time / distance, in-home vs remote services
- Complexity of care (multiple providers, co-morbidities like kidney disease, dementia, etc.)
- Add-on services: fiduciary/trust coordination; legal / financial referrals; transportation of remains; grief / bereavement support
Typical pricing models we may use:
- Initial Assessment Fee – A one-time fee to gather all needed information and build care plan.
- Monthly Retainer / Subscription Packages – Different tiers (e.g. Basic, Enhanced, Concierge) depending on service intensity.
- Hourly & A La Carte Add-Ons – For services outside your package: specialty therapies, fiduciary/legal, travel, etc.
- Episode or Project-Based Pricing – For one-time transitions or crises (e.g. rehab post‐surgery, short/long distance relocation, transition to assisted living).

- Initial in-home assessment: ~$500–$800 (based on travel & complexity)
- Basic coordination & monitoring: e.g. $500–$2,000/month
- Enhanced package (more frequent in-home care, more coordination, specialized therapies): $2,500–$4,000/month
- Concierge / full support (24/7 crisis backup, end-of-life planning, specialized equipment/supply management): $$$ higher depending on scope
