Older Oak exists for that moment.
For too long, aging in America has been shaped by fragmentation, fear, and systems that ask families to coordinate what should never have been their burden alone. Adult children find themselves managing care across doctors, hospitals, home health providers, and family dynamics — often while working, parenting, and living at a distance.
At Older Oak, we step into that gap.
Our Patient Care Advocates and Certified Geriatric Care Management Consultants serve as the steady presence families need when aging becomes complex. We coordinate care across nursing, home health, and psychosocial support so older adults can remain safely at home — with dignity, clarity, and continuity.
In 1800s slang, the phrase “I’m your Huckleberry” meant someone was the right person for a difficult task. We take that meaning seriously. We are a group of seasoned social workers, nurses and caregivers with a collective experience that allows us to bring together a uniquely tailored approach to aging needs.
Older Oak was born out of client visits and their life stories- in the hundreds+ and the team meetings following those visits to determine optimal care and the frustrations felt about limitations of patient care. The Older Oak approach to geriatric care is about being involved in the part of Your Life Story where we help you to Age with Resilience.
