Our Story

Our Story

Our Story

We are a group of seasoned social workers, nurses and home health providers. Our collective experience allows us to bring together a uniquely tailored psycho-social approach to specified aging needs. 


Older Oak was born out of client visits and their life stories- in the hundreds+ and the team meetings following those visits to determine optimal care and the frustrations felt about limitations of patient care. The Older Oak approach to geriatric care is about being involved in the part of your Life Story where we help you to Age with Resilience.

Our mission is to guide older adults and their families through the challenges of aging with dignity, independence, and peace of mind. We provide a Continuum of Care that integrates health, emotional, and practical support into one seamless plan. Rooted in person-centered care, we respect each individual’s values and independence while navigating difficult transitions together. Guided by the wisdom of the owl and the strength of the oak, we coordinate across providers, empower families, and ensure that nothing falls through the cracks.
